VA Cemetery Interment

 

NATIONAL CEMETERY ADMINISTRATION PRE-NEED ELIGIBILITY 

VA Form 40-10007 is the Burial Pre-Need Form that is used to determine eligibility.

To process a Pre-Need determination, the following information and documentation is needed: 

The Veteran’s name, social security number, military service number (if applicable), date and place of birth 

If married, spouse’s name Point of contact and telephone number 

Any military discharge documents (i.e. DD 214) or other pertinent military information. 

The information and documentation can be submitted by being dropped off at nearest national cemetery, fax, mail, or e-mail.

Ensure you include a subject line with Pre-Need Eligibility and the last name of the person for whom you are requesting the Pre-Need determination 

The NCA will process the Pre-Need burial eligibility request and provide initial response within 60 days of receipt of the information. Please provide a current mailing address where the determination can be mailed to. If you have any questions please call and speak to any of our Fort Snelling Cemetery Representatives at 612-726-1127. They are available Monday – Friday, 8:00 am to 4:00 pm.
